Stimulant Prescriptions for ADHD Surge by 157% in Ontario
A recent study reveals a significant increase in stimulant prescriptions for treating attention deficit/hyperactivity disorder (ADHD) in Ontario. The research indicates that from 2015 to 2023, the number of new prescriptions for stimulant medications such as Ritalin and Adderall surged by 157 percent. This trend could be attributed to several factors, including increased online activity and the rise of private assessments for ADHD.
Published in JAMA Network Open on March 15, 2024, the study analyzed data from prescriptions dispensed in Ontario over the eight-year period. Notably, the rate of new stimulant prescriptions began to accelerate in 2020, coinciding with the COVID-19 lockdowns. During this year, prescriptions rose by 28 percent annually, compared to a modest 7 percent increase from 2015 to 2019.
Researchers from ICES, North York General Hospital, and The Hospital for Sick Children suggest that the rise in prescriptions may be linked to social changes, including increased screen time and a growing public awareness of ADHD symptoms. The report also highlights the possibility that many individuals who previously went undiagnosed are now seeking help.
While the study provides valuable insights, the researchers acknowledge a gap in data regarding the appropriateness of these prescriptions. Concerns have been raised about potential misdiagnosis or overdiagnosis of ADHD, particularly when other conditions may present similar symptoms.
Lead author Dr. Daniel Myran, a family and public health physician based in Ottawa, expressed his worry that some patients might be exposed to unnecessary side effects from medication. He noted that this could lead to missed opportunities for addressing other mental health issues.
The most significant increase in prescriptions was observed among adult females. The data showed rises of approximately 421 percent for women aged 25 to 44 and 369 percent for those aged 18 to 24. Dr. Myran pointed out that by 2023, there were more females aged 25 to 44 using stimulants than males in the same age group. Historically, ADHD diagnoses have focused on hyperactivity, more prevalent in males, but recent attention to inattentive symptoms in women may be contributing to the increase in diagnoses among females.
The report also highlights a surge in prescriptions for children, although the increase was less pronounced than in adults. Girls aged 10 to 14 saw a 191 percent rise in new prescriptions, compared to a 67.5 percent increase for boys. The pandemic saw a proliferation of private virtual care clinics offering ADHD assessments, which may have lowered barriers to diagnosis but also raised concerns about misdiagnosis.
Dr. Myran emphasized the importance of accurate diagnoses. “If you have the wrong diagnosis and pursue the wrong treatment, you may never receive the appropriate care for your condition,” he stated.
Medication for ADHD can lead to side effects such as loss of appetite, sleep disturbances, and anxiety. Long-term implications may include increased blood pressure and heightened risks of stroke and heart attacks. Dr. Myran noted that for individuals with severe ADHD symptoms affecting their education or work, the risk-benefit analysis may appear clear.
As the use of ADHD medications broadens, he cautioned that the risk-benefit ratio may not remain favorable for all individuals prescribed these drugs.
A similar study conducted in British Columbia corroborated these findings. Heather Palis, a senior scientist at the BC Centre for Disease Control, reported that the rapid rise in new diagnoses could indicate a population previously underdiagnosed and managing symptoms without professional support. Her research, published in The Lancet, found the fastest growth in ADHD diagnoses among females, suggesting a critical need for accessible healthcare services.
Palis emphasized the importance of ensuring that individuals with new diagnoses have access to healthcare providers who can prescribe and monitor treatment effectively. “This is a first step towards meeting the service needs of those with ADHD and addressing the gaps for individuals who may have been overlooked,” she said.
The findings from Ontario and British Columbia highlight a complex landscape of ADHD diagnosis and treatment, revealing both challenges and opportunities in mental health care. As awareness of ADHD grows, so does the need for careful assessment and appropriate treatment strategies to support those affected.
