New Research Reveals Insights on Blood Tests for Alzheimer’s Detection

Recent advancements in blood tests designed to detect biomarkers for Alzheimer’s disease have shown promise for early detection. New research highlights that tracking changes in certain protein levels over time is more effective than relying solely on a single blood test result. This progress aims to create an early warning system for Alzheimer’s, potentially allowing intervention before significant memory decline occurs.

A study presented at the Radiological Society of North America’s annual meeting in March 2023 indicates that obesity can obscure signals of Alzheimer’s by diluting proteins associated with the disease. This dilution makes a single blood test less reliable. Additionally, a separate study discovered that individuals who expressed concern about their memory but performed well on cognitive tests showed a significant increase in Alzheimer’s-linked proteins in their blood over five years, despite having normal initial test results.

The findings reflect a significant evolution in the methods used to detect Alzheimer’s. Traditionally, diagnosis relied on brain scans or spinal taps. During a PET scan, a radioactive tracer is injected to highlight amyloid protein clusters in the brain, which are associated with cognitive decline. These tests, however, are often conducted in later stages of cognitive decline and can be expensive and difficult to access due to limited resources and insurance barriers.

Over the past few years, the landscape of Alzheimer’s detection has shifted dramatically due to the development of blood tests. The U.S. Food and Drug Administration (FDA) recently approved two blood tests that measure Alzheimer’s-related biomarkers, which the Alzheimer’s Association has incorporated into clinical guidelines for specialty care settings.

Sheena Aurora, a practicing neurologist and vice president of medical affairs at the Alzheimer’s Association, explained that one test serves as a “rule-out” method for primary care providers, indicating when amyloid buildup is unlikely to be the cause of symptoms. The second test functions as a “rule-in” tool for specialists, suggesting a higher likelihood of the presence of Alzheimer’s-linked proteins in the brain.

Despite the advancements, experts caution against the overuse of these tests. Aurora emphasized that they should complement other diagnostic measures, such as family history and cognitive screenings, rather than serve as standalone tools. Furthermore, these blood tests require clinical supervision by qualified professionals who can accurately interpret the results.

The potential benefit of blood-based tests lies in their ability to detect changes in amyloid and tau proteins through a simple blood draw. Early identification of Alzheimer’s could facilitate timely interventions, possibly slowing the progression of the disease. Raji noted, “Alzheimer’s disease starts 20 plus years before the earliest detectable symptoms show up,” underscoring the necessity of early detection for effective treatment.

Nevertheless, the reliability of blood tests can be compromised by chronic health conditions. A study co-authored by Corey Bolton from Vanderbilt University Medical Center found that kidney function significantly impacts blood test accuracy. Approximately one-third of older adults with mild kidney issues may receive unreliable test results, particularly for the neurofilament light chain biomarker, which indicates nerve damage in the brain.

This challenge highlights a broader issue with blood tests for Alzheimer’s. Unlike PET scans or spinal taps that directly reflect brain changes, blood tests can be influenced by various factors, including kidney disease, obesity, vascular conditions, and certain medications.

Researchers, including Argonde C. van Harten from Amsterdam UMC, advocate for a deeper understanding of how chronic conditions affect blood test results. They emphasize the importance of considering diverse patient populations in biomarker validation studies, as most existing research often includes homogeneous groups, typically comprising White, college-educated individuals.

The importance of longitudinal testing is also emphasized in studies tracking Alzheimer’s biomarkers over time. A study involving 298 participants, who initially displayed normal cognitive function but expressed memory concerns, monitored changes in Alzheimer’s-linked proteins every two years. Researchers documented that a significant increase in tau and glial fibrillary acidic proteins correlated with measurable declines in cognitive function and advances toward mild cognitive impairment or dementia.

Approximately 20% of participants who began with normal biomarker levels later exhibited abnormal levels, according to the study. These findings suggest that repeated blood tests provide a more comprehensive view of cognitive health than a single measurement.

As research into Alzheimer’s blood tests progresses, the potential for early intervention remains a critical focus. The objective is to tailor treatments to individual patients, improving predictive accuracy and outcomes. Future advancements in blood-based biomarkers could pave the way for minimally invasive assessments of biological changes associated with Alzheimer’s, enhancing access to diagnostics for millions globally. As researchers continue to refine these tests, the hope is to better equip healthcare providers in the fight against this devastating disease.
